About the Role

As a Physical Therapist at Highland Care Center, you will be responsible for providing high-quality, patient-centered care to individuals with various medical conditions and injuries. You will work closely with our rehabilitation team to develop and implement personalized treatment plans, conduct assessments, and progress patients towards their functional goals.

Responsibilities:

• Conduct initial and ongoing assessments to identify patient needs and develop individualized treatment plans
• Design and implement treatment programs, including exercises, modalities, and other interventions to achieve patient goals
• Collaborate with interdisciplinary team members to ensure patient care is coordinated and effective
• Provide education and training to patients, families, and caregivers on proper techniques and self-care strategies
• Maintain accurate and detailed records of patient progress, including assessments, treatments, and outcomes
• Participate in quality improvement initiatives to ensure high-quality care and optimal patient outcomes
