Position Summary

The Physical Therapist provides skilled physical therapy services to residents in a skilled nursing facility (SNF) setting. The PT evaluates, plans, and implements individualized treatment programs to restore, maintain, and promote optimal physical function, mobility, and quality of life. The therapist collaborates closely with an interdisciplinary team to ensure residents achieve their highest functional potential.

Essential Duties and Responsibilities

  • Evaluate residents’ functional status, range of motion, strength, balance, and mobility needs.
  • Develop and implement individualized treatment plans based on clinical findings and resident goals.
  • Provide direct physical therapy interventions, including therapeutic exercises, gait training, balance and coordination activities, and pain management techniques.
  • Document evaluations, progress notes, and discharge summaries in accordance with state, federal, and facility guidelines.
  • Monitor residents’ responses to treatment and modify care plans as necessary.
  • Educate residents, families, and caregivers on home exercise programs, adaptive equipment, and safety.
  • Participate in interdisciplinary team meetings and contribute to care planning discussions.
  • Supervise and mentor Physical Therapist Assistants (PTAs) and collaborate with other rehabilitation staff.
  • Ensure compliance with all regulatory requirements and professional standards of practice.

Qualifications

  • Education: Graduate of an accredited Physical Therapy program (DPT, MSPT, or BPT).
  • Licensure: Current, active state license to practice Physical Therapy.
  • Experience: Prior experience in a skilled nursing facility or geriatric rehabilitation preferred.

Physical Requirements

  • Ability to lift, move, and transfer residents safely.
  • Frequent standing, walking, bending, and reaching.
  • May be exposed to infectious materials and chemicals consistent with a healthcare environment.
