About this role

Position: FQHC Nurse Manager, Onsite in Hyannis MA- Full Time

Company Overview

Duffy Health Center provides compassionate, integrated healthcare for individuals experiencing homelessness, housing instability, and complex medical and behavioral health needs on Cape Cod.

As a Federally Qualified Health Center (FQHC), Duffy Health Center delivers care through interdisciplinary, trauma-informed, and patient-centered models. Our staff are deeply committed to equity, dignity, and innovation in healthcare delivery. Many team members bring lived experience and a strong dedication to serving vulnerable populations.

Position Summary

The Nurse Manager provides clinical, operational, and administrative leadership for nursing and assigned allied health personnel at Duffy Health Center. Reporting directly to the Director of Nursing & Allied Health, the Nurse Manager leads daily staffing, clinical workflow execution, employee supervision, competency development, and nursing quality and safety within the Patient-Centered Medical Home (PCMH) model.

This role translates divisional priorities into consistent frontline practice and works closely with Medical Leadership, Practice Operations, Quality, and specialized care management programs. The Director retains accountability for divisional strategy, program development, budget authorization, and executive oversight.

Essential Duties and Responsibilities

· Direct daily nursing and assigned clinical support operations, including staffing coverage, assignments, patient flow, and escalation of clinical concerns.

· Supervise RNs, LPNs, Medical Assistants, and other assigned personnel; ensure duties align with licensure, training, delegation requirements, and organizational policy.

· Coordinate operational response to absences, high-volume periods, walk-ins, and urgent patient needs; provide direct clinical support when necessary.

· Partner with providers and Practice Operations to maintain safe, timely, equitable patient access.

Workforce Management & Accountability

· Lead operational interviews and recommend hiring decisions; coordinate orientation and performance documentation with Human Resources.

· Review and approve timecards and PTO requests within delegated authority; reconcile attendance discrepancies and coordinate payroll deadlines.

· Manage staff schedules, time and attendance, coverage planning, and approved leave within established policies.

· Participate in hiring, onboarding, coaching, annual evaluations, performance improvement, corrective action, and retention with Human Resources.

· Monitor staffing productivity, overtime, and workload; elevate staffing needs and recommendations to the Director.

· Conduct regular staff meetings, rounding, and individual check-ins to reinforce expectations and employee engagement.

PCMH & Standardized Clinical Workflows

· Implement approved clinical policies and procedures, train staff on revisions, monitor adherence, and recommend needed changes to the Director and appropriate medical leadership.

· Monitor frontline adherence to after-hours clinical coverage, telephone triage, follow-up, clinical emergency, and staff emergency procedures; recommend revisions for Director and medical leadership approval.

· Support PCMH evidence collection, nursing workflow readiness, and annual recognition activities while executive certification accountability remains with designated organizational leadership.

· Oversee consistent execution of daily huddles, pre-visit planning, MA rooming, RN co-visits, and chronic disease management workflows.

· Ensure clear role delineation and adherence to approved telephone triage, same-day nursing access, patient outreach, and Epic documentation procedures.

· Support preventive screening, care gap closure, referral follow-up, and transitions of care in partnership with Population Health and Care Management.

· Identify operational barriers and implement approved workflow changes with measurable follow-up.

Clinical Quality, Safety & Compliance

· Coordinate medical supply inventory and ordering with the Practice Operations Manager within approved purchasing and budget parameters.

· Serve as operational lead for immunization clinic workflows and the state-supplied vaccine program, including inventory reconciliation, storage-temperature monitoring, vaccine accountability, and required Massachusetts DPH documentation and reporting, in coordination with designated program and medical leadership.

· Coordinate day-to-day infection prevention activities, including annual influenza vaccination compliance tracking, infection control audits, staff education, and patient-facing communications under approved policy.

· Coordinate routine clinical equipment readiness, preventive maintenance follow-up, and documented checks with appropriate facilities or vendor personnel.

· Track reported clinical incidents, support investigations and trend review, and verify completion of approved corrective actions; promptly escalate serious events.

· Monitor nursing documentation, medication administration, infection prevention, clinical protocols, and patient safety practices.

· Perform routine chart reviews, direct observations, audits, and corrective action follow-up in collaboration with the Quality Department.

· Escalate incidents, scope-of-practice concerns, and high-risk clinical events promptly through established reporting channels.

· Support HRSA, PCMH, OSHA, Massachusetts nursing regulatory requirements, and applicable organizational readiness activities.

Orientation, Education & Competencies

· Coordinate nursing student clinical rotations, preceptor assignments, and onboarding requirements with Cape Cod Community College and other approved academic partners; escalate affiliation-agreement decisions to the Director.

· Serve as the nursing and clinical-support Epic workflow resource or coordinate designated super-users; validate staff EHR proficiency and correct documentation variation with IT and Quality.

· Lead standardized onboarding, annual competency validation, and role-specific training for assigned clinical staff.

· Maintain training records and monitor licensure, certifications, BLS, and required education completion.

· Identify learning gaps from audits and performance reviews; coordinate remediation and professional development.

· Support student placements and nursing apprenticeship activities as assigned.

Performance Improvement & Resource Stewardship

· Coordinate nursing-related Environment of Care rounds, track deficiencies, and ensure timely operational follow-up with facilities, safety, and infection prevention partners.

· Identify gaps in frontline nursing services and recommend improvements or potential service opportunities to the Director; do not independently authorize new service lines.

· Track nursing-sensitive measures, workflow compliance, staffing trends, patient access, documentation quality, and assigned performance indicators.

· Lead nursing-focused Plan-Do-Study-Act improvement cycles and report progress to the Director.

· Support accurate capture and documentation of eligible nursing visits and efficient use of clinical supplies.

· Submit routine operational summaries with risks, corrective actions, and resource recommendations.

Interdisciplinary Collaboration & Leadership

· Coordinate assigned nursing support for Medical Respite and specialty programs without assuming independent program-director authority, admissions governance, or supervision of separately assigned program staff.

· Represent nursing operations on assigned Environment of Care, PCMH, Epic, Clinical Integration, Workplace Safety, and operational workgroups; committee chairing and enterprise governance remain as delegated by the Director.

· Act as an essential on-site clinical operations leader during declared emergencies when designated under the Duffy Emergency Operations Plan, coordinating nursing coverage and escalation through incident command.

· Participate in medical, nursing, quality, and care team meetings; communicate decisions and expectations to frontline staff.

· Coordinate with OBAT, Medical Respite, Population Health, Complex Care Management, Behavioral Health, and other program leaders without superseding their designated program authority.

· Implement approved policies and organizational changes; escalate decisions involving new positions, material budget changes, and divisional strategy to the Director.
