Register Nurse (RN) Home Care Agency

Overview

As a Registered Nurse (RN) with a home care agency, you’ll deliver compassionate, skilled care to clients in their homes. You’ll assess each client’s needs, coordinate care with healthcare professionals, and maintain accurate records while following health regulations and the Health Insurance Portability and Accountability Act (HIPAA).

Duties

• Assess clients, monitor vital signs, document changes, and support diagnostic evaluation and care planning.
• Coordinate intake, clinical case management, utilization management, and discharge planning with clients, families, and healthcare professionals.
• Provide or coordinate home health, hospice, post-acute, and other nursing care within your scope and applicable policies.
• Maintain electronic health records (EHR), medical records, and clear client communications while protecting confidentiality.
• Support continuity of care across primary care, hospital, clinic, and outpatient settings.

Skills

• Nursing experience and knowledge of patient care, patient assessment, physiology, medical terminology, and patient service are essential.
• Familiarity with home health, case management, managed care, health regulation policy, and EHR management is valuable.
• Experience in hospital, inpatient, acute care, medical-surgical, emergency room, emergency medicine, intensive care unit (ICU), critical care, or Level I or II trauma settings is relevant.
• Additional experience in pediatrics, pediatric intensive care unit (PICU), operating room (OR), geriatrics, behavioral health, infectious disease care, hospital medicine, or hospice is welcome.
• Knowledge of utilization management and experience as a nurse case manager are helpful; experience in post-acute care, discharge planning, intake, or clinic settings is also relevant.

Pay: $35.00 - $40.00 per hour
