Concierge Home Care is hiring a **Home Health Physical Therapist (PT)**. **Your Role as a Home Health Physical Therapist*** Evaluate patients and develop individualized treatment plans

• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
