Concierge Care Collective has coordinated private behavioral health care for individuals. We work with families, family offices, and their advisors to build and carry out care plans for individuals facing complex mental health and substance use challenges. Our companions cover the in-between times: the hours and days outside a clinician’s office, where progress either takes hold or slips.

Role 1

Live-In Nurse Companion (24/7)

Location: Los Angeles

Type: Independent contractor (1099)

Pay: Flat day rate, [range]

Schedule: Live-in assignments on a rotation of 7 days on and 7 days off, 14 days on and 14 days off, or 25 days on and 5 days off

The Role

We are building a team of nurses who want to practice differently. As a Live-In Nurse Companion, you live alongside one individual around the clock, in their home or while they travel, and you are the steady presence through every part of the day. Mornings, evenings, the overnight hours, and the moments in between.

This is not about supervision. It is about activation.

You bring clinical judgment to a nonclinical setting. You notice what others miss, you stay steady in a crisis, and you keep the care team informed.

What You Will Do

• Live on site with one individual and provide round-the-clock companion support for the full assignment
• Build and protect a daily rhythm: sleep, meals, movement, appointments, and structure
• Support medication adherence and the treatment plan set by the individual’s clinicians
• Observe and report symptoms, behaviors, and changes in functioning to the individual’s treating therapists and psychiatrists, and to our clinical partners
• Respond calmly to crises at any hour and escalate to the care team as directed
• Travel with the individual when the care plan calls for it
• Write a daily report for the care team
• Coordinate with the case manager and family as directed
• Represent Concierge Care Collective with discretion in private homes, hotels, and professional settings

What We Are Looking For

• Active RN, LPN, or NP license in California; multistate compact license a plus
• At least 2 years of experience in behavioral health
• Comfortable living in a private home for extended periods while keeping firm professional boundaries
• Calm under pressure, including overnight, and without a hospital structure around you
• Polished, discreet, and at ease with high-net-worth families
• Available for travel
• Willingness to complete a background check, education verification, and professional and personal references

Role 2: Nurse Coach (Hourly)

Location: Los Angeles

Type: Independent contractor (1099)

Pay: $75 to $115 per hour

Schedule: Scheduled sessions built around the individual’s care plan and your availability

The Role

We are building a team of nurses who want to practice differently. As a Nurse Coach, you meet with individuals in scheduled sessions, in their home, at their work, or out in the community, and help them build the daily rhythm that stability depends on. You help them act their way into better thinking: one routine, one appointment, one good day at a time.

This is not about supervision. It is about activation.

You bring clinical judgment to a nonclinical setting. You notice what others miss, you stay steady in a crisis, and you keep the care team informed.

What You Will Do

• Provide scheduled one-to-one coaching sessions with individuals who have complex mental health needs
• Help individuals build routines, keep appointments, and carry out the goals in their care plan
• Support medication adherence and the treatment plan set by the individual’s clinicians
• Observe and report symptoms, behaviors, and changes in functioning to the individual’s treating therapists and psychiatrists, and to our clinical partners
• Write a report for the care team after each session
• Coordinate with the case manager and family as directed
• Represent Concierge Care Collective with discretion in private homes, workplaces, and public settings

What We Are Looking For

• Active RN, LPN, or NP license in CA; multistate compact license a plus
• At least 2 years of experience in behavioral health
• Calm under pressure and comfortable working without a hospital structure around you
• Polished, discreet, and at ease with high-net-worth families
• Reliable transportation within Los Angeles
• Willingness to complete a background check, education verification, and professional and personal references

What We Offer

• $75 to $115 per hour
• Mileage reimbursed when driving with the individual (travel to and from sessions is not covered)
• A flexible schedule built around your availability
• Full training through our field training program

Pay: $65.00 - $90.00 per hour

Expected hours: 10.0 – 50.0 per week
