Physical Therapist - Home Care - Westchester County

Compass Healthcare Consulting & Placement is conducting a search for a Certified Home Health Care Agency seeking a Full Time Physical Therapist, PT, to conduct visits in Westchester County, NY . Physical Therapist also needed to make visits in the Bronx, NY. Competitive Salary Plus Excellent Benefits!

Must be Licensed in NYS as a Physical Therapist, must have reliable vehicle for travel.

Responsibilities

Include:

Visiting and Providing Physical Therapy Home Health Care Services for patients in the community.

Full Time PT role makes 30 visits per week.

Assessing patients, creating and implementing plan of care for Home Care patients

All documentation is Computerized.

Qualified candidates must have driver's license and reliable vehicle to make patient visits in Westchester County, NY. Additional opening for Physical Therapist to make patient visits in the Bronx, NY.

Requirements:

• CHHA experience preferred but not required
• Valid Physical Therapist license required
• Minimum 1 year of post-graduate work experience
• Must have a reliable vehicle and be willing to drive

Competitive Salary & Benefits! Salary $115,000 - $120,000 based on experience, plus benefits
