PHYSICAL THERAPIST HOME HEALTH

Overview

As a Physical Therapist in home health, you’ll help patients build strength, mobility, and independence where they live. You’ll assess each patient’s needs, develop individualized care plans, and deliver physiotherapy that supports safe, meaningful progress. Your work may include home and community care, patient rehabilitation, and coordination with patients’ healthcare teams.

Duties

• Evaluate patient function and mobility, applying knowledge of anatomy, physiology, orthopedics, neurology, and medical terminology.
• Develop and update treatment plans and care plans; provide physical therapy interventions, including trigger point therapy when appropriate.
• Deliver patient care for adults, including geriatric patients, and for pediatric patients as assigned.
• Collaborate with healthcare professionals, patients, and caregivers; communicate goals, progress, and discharge plans clearly.
• Maintain accurate physical therapy documentation in electronic health record (EHR) or electronic medical record (EMR) systems, following clinical documentation standards.

Qualifications

• Experience practicing in a physical therapy role and the ability to provide safe, patient-centered care in home health settings.
• Knowledge of patient assessment, treatment plan development, rehabilitation, and physical therapy documentation.
• Experience with geriatric care is valuable. Acute care, inpatient, hospital, outpatient, clinic, outpatient rehabilitation center, or home and community care experience is also welcome.
• Familiarity with EHR or EMR management and applicable clinical documentation standards.
• Strong client communication skills and the ability to coordinate care with healthcare professionals.

Pay: $130.00 - $150.00 per hour
