Collective Therapy is seeking a compassionate and motivated PRN Physical Therapist to join our team at Southminster Retirement Community, one of Charlotte's premier Continuing Care Retirement Communities (CCRC).

About the Position
This Physical Therapist will provide individualized evaluations and treatment services to residents throughout Southminster's beautiful continuing care retirement community. You'll work with individuals across independent living, assisted living, healthcare, and memory care, helping them improve mobility, strength, balance, safety, and overall quality of life.
This position is ideal for a therapist who enjoys building lasting patient relationships and practicing evidence-based, personalized care.
Responsibilities
- Perform comprehensive physical therapy evaluations
- Develop individualized plans of care
- Provide skilled therapeutic interventions
- Collaborate with physicians, nursing staff, and interdisciplinary team members
- Complete timely and accurate documentation
- Educate patients and caregivers on home exercise programs and fall prevention
- Participate in team meetings and care planning as needed
Qualifications
- Licensed Physical Therapist in North Carolina
- Excellent communication and interpersonal skills
- Strong clinical reasoning and patient-centered approach
- Experience in geriatrics, outpatient, skilled nursing, or CCRC settings is preferred
