Choice Health at Home is seeking a compassionate PRN Physical Therapist to provide personalized care that improves physical function and quality of life.

What You’ll Do

• Complete and document initial evaluations to identify patient needs and establish individualized treatment plans.
• Provide therapeutic interventions to improve mobility, relieve pain, increase strength, and restore function.
• Monitor patient progress and adjust treatment plans to support the best possible outcomes.
• Educate patients and caregivers on exercises, safety, and strategies for continued recovery.
• Collaborate with families and the healthcare team to ensure continuity of care.
• Follow infection control practices, agency policies, and patient care standards.
• Complete accurate documentation within company guidelines.

What You’ll Bring

• Graduation from an accredited Physical Therapy program and successful completion of the state licensure exam.
• A valid Physical Therapy license.
• Current CPR certification.
• A valid driver’s license and auto insurance.
• One year of home health experience preferred.
• Homecare Homebase experience preferred.

Physical Requirements

• Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Ability to lift, carry, pull, or push up to 100 pounds.
