**Position Summary**

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

As a **Physical Therapist**, you will:* Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

**Requirements*** Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

**Physical Demands*** Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Must be able to lift, carry, pull, or push up to 100 lbs..
