About this role

Chestnut Health is a multi-specialty healthcare organization focused on rehabilitative care and physical medicine and rehabilitation. We support patients through specialized care services and are building an HR function that can help the organization operate effectively, support employees well, and scale with the needs of the business.

We are seeking an experienced HR Manager to lead all human resources operations from our Oak Brook, IL office. This is a hands-on generalist role for someone who can build and run HR across recruitment, benefits, payroll, retirement plan administration, leave management, and company policy.

The ideal candidate brings deep experience in healthcare provider organizations and thrives in a start-up environment where priorities shift and processes are still being built.

Key Responsibilities

HR Operations & Leadership

  • Lead day-to-day HR operations and serve as the primary HR resource for leadership and employees
  • Build scalable HR processes, systems, and workflows to support company growth
  • Maintain accurate employee records and HRIS data

Recruitment & Onboarding

  • Oversee recruitment for clinical and non-clinical roles in partnership with the recruiting team and hiring managers
  • Manage offer letters, background checks, credentialing coordination, and new hire onboarding

Benefits & 401(k) Administration

  • Administer health, dental, vision, and other benefit programs, including open enrollment and carrier relationships
  • Serve as 401(k) plan administrator: enrollments, contributions, vendor coordination, and annual compliance testing

Payroll

  • Manage multi-state payroll processing, ensuring accuracy, timeliness, and tax compliance
  • Partner with finance on payroll reconciliation and reporting

Leave Management

  • Administer FMLA, ADA accommodations, and state-specific leave programs
  • Coordinate workers' compensation claims and return-to-work processes

Policies & Compliance

  • Develop, update, and maintain the employee handbook and company policy documents
  • Ensure compliance with federal and state employment laws across all states where we operate
  • Advise leadership on employee relations, performance management, and investigations

Requirements Required Qualifications

  • 7+ years of experience leading HR operations
  • 5+ years of HR experience within a healthcare provider organization (e.g., medical practice, physician group, post-acute or long-term care provider). This requirement is firm; candidates without healthcare provider experience will not be considered.
  • Proven generalist experience across recruitment, benefits, payroll, 401(k) administration, leave management, and policy development
  • Working knowledge of federal and multi-state employment law, including FMLA, ADA, FLSA, and COBRA
  • Comfort working in a start-up environment: self-directed, adaptable, and willing to build processes from the ground up
  • Ability to work on-site in Oak Brook, IL

Preferred Qualifications

  • Bachelor's degree in Human Resources, Business Administration, or a related field
  • SHRM-CP, SHRM-SCP, PHR, or SPHR certification
  • Experience supporting a distributed workforce of PAs, NPs, or other clinicians across multiple states
