Cheer Health Care is hiring a passionate Registered Nurse (RN) to deliver bespoke, one-on-one home health services to seniors in our community. Working in a concierge capacity, you will partner alongside an interdisciplinary care team to design, execute, and monitor personalized health programs, serving as a trusted clinical advocate and communication hub for clients, family members, and fellow clinicians.

Core Responsibilities

• Holistic Patient Evaluations: Perform detailed clinical evaluations addressing each individual's physical condition, psychological well-being, and living environment.
• Tailored Care Coordination: Formulate dynamic, evidence-driven care roadmaps with concrete health milestones tailored to every patient's lifestyle and goals.
• Proactive Chronic Disease Oversight: Direct long-term illness management strategies, emphasizing preventative care to avoid hospital readmissions and health declines.
• Skilled Clinical Interventions: Administer high-touch nursing procedures, including surgical aftercare, complex wound dressing, and precise medication administration.
• Patient & Family Guidance: Empower clients and their designated caregivers through clear, practical coaching regarding diagnoses, recovery protocols, and everyday self-management.
• Independence & Safety Advocacy: Foster senior autonomy, fall prevention, and total well-being to maximize comfort and daily quality of life at home.

Requirements

• Current, unrestricted California registered nurse license
• 3 years of experience as a registered nurse
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Active California Drivers license and car insurance
• Cell phone with Internet Access

Hourly Pay Rate: $48-$75 based on experience. Potential for team bonus.
