About this role

Join a Mission-Driven, Recovery-Focused ACT Team

The Center for Health & Recovery (CHR) is seeking a skilled, compassionate, and highly organized Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team serving individuals living with Serious Mental Illness (SMI).

This position is ideal for an RN who thrives in a fast-paced, community-based behavioral health setting and understands the clinical complexity of individuals with co-occurring mental health and substance use conditions. Ideal candidates additionally know there is no "I" in team. We need team players, willing to do what it takes to support our members and program.

Our ACT team recently completed a WICHE Fidelity Review, scoring 4.54 which reflects the teams commitment to excellence and high priority for providing quality care to our members. An ACT Team provides intensive, community-based services following SAMHSA ACT fidelity standards.

The RN plays a critical role in medication management, care coordination, clinical triage, and ensuring safe, effective psychiatric and medical treatment for members served.

About ACT (Assertive Community Treatment)

ACT is an evidence-based, multidisciplinary team model designed to provide comprehensive, community-based psychiatric treatment, rehabilitation, and support to individuals with SMI. Services are delivered in the community rather than a clinic setting and emphasize:

  • Low staff-to-member ratios
  • Shared caseload responsibility
  • 24/7 crisis availability
  • Integrated psychiatric, medical, and substance use care
  • Recovery-oriented, person-centered treatment planning

Position Responsibilities

  • Oversee medication ordering, tracking, storage, and disposal in accordance with protocols
  • Provide medication education to members and families
  • Complete discharge summaries and nursing documentation
  • Monitor vital signs and identify changes in medical or psychiatric condition
  • Conduct clinical triage and determine emergent vs. non-emergent needs
  • Escalate concerns appropriately to prescriber/provider per protocol

Care Coordination

  • Coordinate with psychiatric providers, primary care providers, pharmacies, hospitals, and community partners
  • Participate in daily team meetings and shared treatment planning
  • Assist in transitions of care following hospitalization
  • Support integrated care for members with co-occurring substance use disorders

Compliance & Documentation

  • Maintain timely, accurate, and thorough clinical documentation
  • Follow ACT fidelity standards and agency protocols
  • Adhere to Arizona Nurse Practice Act and professional scope of practice
  • Ensure proper chain of command and escalation procedures are followed

Qualifications

  • Active Arizona RN license (in good standing)
  • Experience in behavioral health or psychiatric nursing preferred
  • Experience with SMI population strongly preferred
  • Knowledge of psychotropic medications and side-effect management
  • Experience administering injectable medications
  • Experience with blood draws and lab coordination
  • Strong clinical judgment and triage skills
  • Ability to manage workload in a community-based setting
  • Excellent documentation and communication skills
  • Valid Arizona driver’s license (community-based services required)
  • Those who identify as having their own lived experience are highly encouraged to apply

Preferred Experience

  • ACT team experience
  • Experience working with individuals with co-occurring substance use disorders
  • Familiarity with AHCCCS/Medicaid behavioral health systems
  • Experience in crisis intervention and de-escalation

What We’re Looking For

We are seeking an RN who:

  • Demonstrates sound clinical judgment
  • Understands emergent vs. non-emergent escalation
  • Maintains strong professional boundaries
  • Communicates clearly and effectively with providers and team members
  • Is organized and able to manage medication protocols with precision
  • Thrives in a collaborative, multidisciplinary team environment
  • Is recovery-oriented and person-centered
  • Willing to do both community based / home visits
  • Looking for long term employment placement within an organization dedicated to excellence (longevity is a hiring consideration)
  • Able to work schedule 4- 10’s likely a Tuesday – Friday schedule.

Why Join CHR?
