As a Psychiatric Mental Health Nurse Practitioner (PMHNP), you will play a vital role in our mission to provide high-quality clinical services to improve the quality of life for our older adult patients.

About the Role

As a PMHNP with CHE Behavioral, you will work in collaboration with a multidisciplinary team of healthcare professionals to provide behavioral health services to residents of long-term care and rehabilitation centers. You will assess, diagnose, and treat mental health conditions, as well as develop and implement individualized treatment plans to meet the unique needs of each patient.

Responsibilities

• Assess and diagnose mental health conditions in older adult patients
• Develop and implement individualized treatment plans to meet the unique needs of each patient
• Provide evidence-based treatment interventions, including medication management and therapy
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive care
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and timely records of patient care and treatment
• Participate in staff development and training activities to stay current on best practices in mental health care
