Job Summary

We are seeking a compassionate, autonomous, and skilled Visiting Nurse Practitioner (NP) to provide high-quality, patient-centered care to individuals in their homes or community settings. This PRN role is perfect for a clinician seeking flexibility and the opportunity to make a meaningful impact on patients across the San Diego, Corona, Riverside, and Bakersfield regions.

You will manage a flexible caseload, conducting comprehensive health assessments, managing chronic conditions, and coordinating care to improve patient outcomes and minimize unnecessary hospitalizations.

Key Responsibilities

• In-Home Evaluations: Conduct comprehensive history and physical assessments for patients within their homes or residential facilities.
• Care Management: Develop, implement, and evaluate personalized plan of care adjustments based on clinical findings.
• Chronic Disease Management: Educate patients and caregivers on disease processes, medication adherence, and self-care strategies.
• Collaboration: Coordinate closely with primary care physicians, specialists, and community health resources to ensure continuity of care.
• Documentation: Complete accurate, timely electronic clinical documentation matching all regulatory and billing guidelines.

Qualifications

• License: Current, Nurse Practitioner (NP) license in the State of California.
• NPI: A valid National Provider Identifier (NPI).
• Insurance: Valid insurance
• Experience: Prior experience in home health, visiting nurse roles, geriatrics, or hospice/palliative care is strongly preferred.
• Travel: Reliable transportation, a valid CA driver’s license, and clean driving record are required to travel between patient locations.
