Physical Therapist (PT) – Home Health

Carewell Health Services, Inc. is seeking a compassionate and dependable Physical Therapist (PT) to provide personalized care to patients in their homes. Help patients improve strength, mobility, balance, and independence while working with a supportive home health team.

Duties and Responsibilities

• Perform initial evaluations, reassessments, and discharge assessments.
• Assess strength, balance, mobility, endurance, pain, and functional limitations.
• Develop individualized treatment plans and measurable goals in coordination with the physician and care team.
• Provide therapeutic exercises, gait training, balance activities, transfer training, and other appropriate physical therapy interventions.
• Teach patients and caregivers safe mobility techniques, fall prevention, and proper use of walkers, canes, and other assistive devices.
• Establish and update home exercise programs based on patient needs and progress.
• Monitor treatment response and communicate changes in condition to the physician and care team.
• Supervise Physical Therapist Assistants as assigned.
• Complete accurate and timely evaluations, visit notes, care coordination, and discharge documentation.
• Participate in interdisciplinary care planning to support patient safety and continuity of care.

Qualifications

• Active Nevada Physical Therapist license.
• Graduate of an accredited physical therapy program.
• Current CPR/BLS certification.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Strong clinical assessment, communication, and time-management skills.
• Ability to work independently in patients’ homes.
• Home health experience preferred.
• Familiarity with WellSky/Kinnser is a plus.

• Work setting: In-person visits in patients’ homes.

• Compensation: Based on experience and the agreed position structure.
