We are currently seeking a full-time Physical Therapist (PT) to join a collaborative rehabilitation team serving patients through a combination of inpatient, outpatient, and home health services within a community-based hospital setting.

This position offers a diverse caseload spanning neurological and orthopedic diagnoses and provides the opportunity to work with patients across the lifespan, from school-aged children to geriatric adults.

Position Summary

The Physical Therapist evaluates and treats patients across inpatient, outpatient, and home health settings to improve mobility, restore function, reduce pain, and enhance overall quality of life. Working as part of a multidisciplinary healthcare team, the PT will provide individualized care to a diverse patient population with a wide range of neurological, orthopedic, post-surgical, and medical conditions.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations for patients across inpatient, outpatient, and home health settings.
  • Develop individualized treatment plans based on clinical findings, physician referrals, and patient goals.
  • Treat a diverse caseload including neurological and orthopedic conditions.
  • Provide evidence-based interventions including therapeutic exercise, gait training, balance training, neuromuscular re-education, manual therapy, and functional mobility training.
  • Educate patients and caregivers on home exercise programs, safety strategies, injury prevention, and functional independence.
  • Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals to coordinate care.
  • Complete accurate and timely documentation in accordance with facility, state, and regulatory requirements.
  • Participate in interdisciplinary meetings, discharge planning, and care coordination activities.

Qualifications

Required

  • Doctor of Physical Therapy (DPT) or Master's Degree in Physical Therapy from an accredited program.
  • Current and active state Physical Therapy license or eligibility for licensure.
  • Strong communication, organizational, and clinical reasoning skills.

Preferred

  • Experience in inpatient, outpatient, hospital, rehabilitation, or home health settings.
  • Experience treating neurological and orthopedic diagnoses.
  • Knowledge of EMR documentation systems.
