Full-Time | $38. 00–$55. 00/hr (DOE)

Position Overview
Career Links is seeking a compassionate and dedicated Physical Therapist (PT) to join a skilled nursing facility rehabilitation team. In this role, you will help residents regain mobility, improve strength and balance, maximize functional independence, and enhance their overall quality of life.
This position is ideal for a therapist who enjoys working with the geriatric population and values collaboration within an interdisciplinary healthcare team.
Responsibilities
- Evaluate residents and develop individualized treatment plans based on their functional goals and clinical needs
- Provide evidence-based physical therapy interventions, including therapeutic exercise, gait training, balance training, mobility training, and neuromuscular re-education
- Assist residents in improving transfers, ambulation, strength, endurance, and overall functional mobility
- Collaborate with occupational therapists, speech-language pathologists, nursing staff, physicians, and other healthcare professionals to coordinate patient care
- Educate residents, families, and caregivers on safety strategies, fall prevention, and home exercise programs
- Participate in interdisciplinary care conferences, treatment planning, and discharge planning
- Complete timely and accurate documentation in accordance with facility, state, and federal guidelines
- Maintain compliance with all regulatory and professional standards
Qualifications
- Current Iowa Physical Therapist license or eligibility for licensure
- Graduate of an accredited Physical Therapy program recognized by the Commission on Accreditation in Physical Therapy Education (CAPTE)
- Eligible for or holding certification through the Federation of State Boards of Physical Therapy (FSBPT)
- Strong clinical, communication, and documentation skills
- Experience in skilled nursing, rehabilitation, long-term care, or geriatric settings preferred, but not required
Physical Requirements
- Ability to assist residents with mobility, transfers, and therapeutic activities
- Frequent standing, walking, bending, reaching, and lifting throughout the workday
- Ability to lift up to 50 pounds with assistance as needed
