About the Role

As an Adult Nurse Practitioner with Carease Health - WI, you will play a crucial role in our comprehensive preventative care program, which incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). You will work alongside our experienced clinical team to provide proactive care to patients, focusing on both physical and mental health.

This role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities
