Job Summary

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

The ideal candidate will have a proven track record of building strong relationships with facilities, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities:

• Deliver high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home
• Collaborate with a dedicated scribe team for efficient documentation
• Build strong relationships with facilities, maintaining a patient-centered approach
• Demonstrate excellent communication, collaboration, and teamwork skills
• Stay up-to-date with industry developments and best practices in geriatric care and internal medicine

Requirements:

• Active NP or PA license in the state of practice
• 1+ year of experience in geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
