We are seeking compassionate, motivated Nurse Practitioners to conduct Annual Wellness Visits and comprehensive health assessments in patients' homes. What You'll Do

• Conduct comprehensive, face-to-face Annual Wellness Visits and health assessments in patients' homes.
• Review patients' health histories and assess their overall health status.
• Educate patients on preventive care services and the importance of staying current with recommended screenings.
• Identify potential gaps in care and opportunities for improved health outcomes.
• Complete accurate and timely documentation using the provided electronic medical record (EMR) system.
• Help patients better understand their healthcare benefits and available preventive services.

These visits are consultative and prevention-focused, allowing you to spend meaningful time with patients while supporting better long-term health outcomes.

What We're Looking For

• Active Nurse Practitioner license in the state of AZ.
• Board Certification as a Nurse Practitioner.
• Active National Provider Identifier (NPI).
• Previous wellness assessment or Annual Wellness Visit experience preferred (at least 3 months is a plus).
• Familiarity with electronic medical records (EMR) and clinical documentation.
• Ability and willingness to travel 20-25% within AZ.
• Strong communication skills and a compassionate, patient-centered approach.
• Ability to work a consistent Monday–Friday, 40-hour schedule.
• Bilingual/Spanish-speaking candidates are a plus!
