**Pay:** $58–$65 per hour, based on experience

Care Connection Rehabilitation Center is seeking a compassionate and skilled **Physical Therapist (PT)** to join our interdisciplinary rehabilitation team. This PRN opportunity is ideal for a PT who wants to make a meaningful impact while providing high\-quality care in a supportive skilled nursing environment.

### **Responsibilities**

• Evaluate patients and assess physical conditions, mobility, and functional limitations
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, stretching, and hands\-on treatment
• Monitor patient progress and modify treatment plans as needed
• Document evaluations, treatments, and patient progress accurately
• Educate patients and families on therapy goals, home exercises, and recovery
• Collaborate with nursing and other members of the interdisciplinary care team

### **Qualifications**

• Doctor of Physical Therapy (DPT) from an accredited program
• Current New Jersey Physical Therapist license
• Minimum 2 years of experience preferred
• Skilled nursing or nursing home experience preferred
• Strong clinical assessment, communication, and interpersonal skills
• Familiarity with **SigmaCare and Net Health** preferred
