Care Central Professionals Home Health is seeking a dedicated Occupational Therapist (OT) to join our compassionate and patient-focused team in AUBURN, CA. Responsibilities:
- Conduct skilled OT evaluations and develop individualized care plans.
- Provide interventions to improve safety, independence, and quality of life.
- Educate patients, caregivers, and families on adaptive strategies, equipment use, and home safety.
- Collaborate with our interdisciplinary team to ensure holistic, patient-centered care.
- Maintain accurate, timely, and compliant documentation.
Qualifications:
- Current California OT license in good standing.
- Graduate of an accredited Occupational Therapy program.
- At least 1 year of clinical experience (home health preferred).
- Excellent communication and organizational skills.
- Valid driver’s license and reliable transportation.

Qualifications:
- Current California OT license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Minimum of 1 year of clinical experience (home health experience preferred).
- Strong communication and documentation skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation.
