Schedule: Monday–Friday, 8:00 AM–4:40 PM

Guaranteed Hours: 40 Hours/Week

Job Description

We are seeking an experienced Registered Nurse (RN) Case Manager for a 13-week assignment in Fort Myers, Florida.

The RN Case Manager will coordinate patient care, facilitate discharge planning, collaborate with healthcare professionals, and ensure patients receive appropriate care and resources throughout their healthcare journey.

Responsibilities

• Coordinate patient care and develop individualized care plans.
• Conduct patient assessments and evaluate healthcare needs.
• Facilitate discharge planning and coordinate post-acute care services.
• Collaborate with physicians, nurses, and multidisciplinary healthcare teams.
• Identify barriers to discharge and coordinate appropriate solutions.
• Monitor patient progress and ensure effective utilization of healthcare resources.
• Communicate with patients and families regarding care plans.
• Maintain accurate clinical documentation.

Requirements

• Active Registered Nurse (RN) License – Required.
• Minimum 2 years of Case Management experience – Required.
• BLS certification – Required.
• Accredited nursing degree OR minimum 5 years of relevant experience.
• Strong knowledge of care coordination and discharge planning.
• Excellent communication and organizational skills.
• Flexibility to float to other nearby locations as needed.

• 40 guaranteed hours per week

• Floating to other nearby healthcare locations may be required.
• Travel candidates only.
• 50-mile radius restriction applies.
