**Job Summary**
Brightstar care is seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will provide high\-quality care to patients around the Sacramento, Elk Grove, Rancho Cordova and surronding areas.
The Physical Therapist will be responsible for assessing patient needs, developing individualized treatment plans, performs interventions aimed at improving and enhancing the patient's well\-being and evaluates the patient's progress
**Responsibilities**
- Provide high\-quality physical therapy to patients with diverse diagnoses in a home setting, following the physician’s plan of treatment.
- Develop a plan of Care with the patient and family.
- Collaborate with physicians, families, and other disciplines to develop personalized care plans.
- Communicate with office, physician and pharmacy as needed.
**Qualifications**
- Current and unrestricted PT license in CA
- CPR and TB skin test
- Excellent interpersonal skills
- Valid driver's license, reliable car with auto insurance
- Proof of eligibility to work in the US
- Willing to submit to a background check and drug screen.

Pay: $100\. 00 \- $150\. 00 per hour
