Brightbridge Home Care is seeking a dependable and compassionate Registered Nurse (RN) to join our growing home care team in Indiana. This PRN position is designed to provide support as needed by completing in-home client assessments throughout Central Indiana and surrounding areas.

We are dedicated to helping seniors and individuals remain safe, comfortable, and independent in their own homes while receiving high-quality, personalized care. • Complete initial in-home client assessments and reassessments
• Evaluate client needs and assist with care planning recommendations
• Document assessment findings accurately and in a timely manner
• Communicate assessment outcomes with clients, families, and the care team
• Monitor and report changes in client condition or care needs
• Maintain compliance with company policies and applicable regulations
• Travel throughout Central Indiana and surrounding communities as needed

Qualifications

• Active Registered Nurse (RN) license in the State of Indiana
• Current CPR certification
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• Strong assessment, documentation, and communication skills
• Compassionate, dependable, and professional attitude
• Home care experience preferred but not required
