Duties

• Conduct thorough assessments of patients' health conditions and needs.
• Develop and implement individualized care plans based on patient assessments.
• Administer medications and treatments as prescribed, including injections and tube feeding.
• Monitor vital signs and document changes in patients' conditions.
• Provide specialized care for patients with specific needs such as dementia care, or ostomy care.
• Educate patients and their families about health management and disease prevention.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate medical records in compliance with ICD-10 coding standards and Medicare guidelines.

Skills

The successful candidate will demonstrate the following skills:

• Proficient in dementia care, wound care, and chronic disease management techniques and strategies.
• Proficient in post-operative and post-acute inpatient care.
• Experience in administering injections safely and effectively.
• Knowledge of tube feeding procedures.
• Ability to assess and monitor vital signs accurately.
• Familiarity with ostomy care practices.

Pay: $80,000.00 - $100,000.00 per year
