**Location:** Toledo, Ohio \& Surrounding Areas

**Position:** Occupational Therapist – Home Health

**Schedule:** Flexible / Per Visit

**Blessed Home Health Care Agency** is looking for a compassionate, dependable, and motivated **Occupational Therapist (OT)** to join our growing home health care team. The Occupational Therapist will provide skilled therapy services to patients in the comfort of their homes while helping them improve safety, independence, and quality of life.

Responsibilities

• Perform occupational therapy evaluations and reassessments.
• Develop and implement individualized plans of care.
• Assist patients with improving independence in activities of daily living (ADLs).
• Provide therapeutic exercises and functional training.
• Provide patient and caregiver education.
• Evaluate home safety and recommend adaptive equipment when appropriate.
• Communicate patient progress and changes in condition with physicians and the interdisciplinary team.
• Complete clinical documentation accurately and timely.
• Coordinate care with nursing, physical therapy, speech therapy, home health aides, and other members of the care team.
• Follow physician orders, agency policies, and applicable home health requirements.

Qualifications

• Current **Ohio Occupational Therapist (OT) license** in good standing.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Valid driver's license.
• Reliable transportation and current automobile insurance.
• Ability to travel to patients' homes throughout the service area.
• Home health experience preferred.
