Home Health Registered Nurse

Job Summary

We are seeking a compassionate and reliable Registered Nurse (RN) Field Nurse to provide skilled nursing care to patients in their homes and community settings. The RN will work independently while coordinating care with physicians, patients, families, and the healthcare team.

Job Duties & Responsibilities

• Perform patient assessments and monitor changes in condition.
• Develop and follow individualized plans of care.
• Provide skilled nursing services, treatments, medication administration, and wound care as ordered.
• Educate patients and caregivers on medications, disease management, safety, and care plans.
• Communicate patient status and concerns with physicians and the care team.
• Maintain accurate and timely clinical documentation.
• Coordinate care with other healthcare professionals and service providers.
• Follow infection control, patient safety, HIPAA, and regulatory requirements.
• Provide compassionate, professional, patient-centered care during field visits.

Qualifications

• Current, unrestricted RN license.
• Prior nursing experience preferred; home health or field experience is a plus.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work independently and travel to patient locations.

Pay: $75.00 per hour
