Job Summary:

We are seeking a highly motivated and compassionate Physical Therapist (PT) to join our team at Avista Nursing and Rehab in Saginaw, Michigan. As a Physical Therapist, you will play a vital role in our patient-centered care team, providing high-quality rehabilitation services to our residents.

Responsibilities:

• Conduct thorough evaluations and assessments of patients' physical abilities and develop individualized treatment plans to achieve their rehabilitation goals
• Develop and implement effective physical therapy interventions, including exercises, stretches, and therapeutic activities, to improve patients' strength, flexibility, and range of motion
• Collaborate with other healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each patient
• Provide ongoing education and training to patients, families, and caregivers on proper transfer techniques, ambulation, and other safety and wellness strategies
• Maintain accurate and detailed records of patient progress and care delivery, in accordance with regulatory requirements and facility policies
• Participate in interdisciplinary team conferences and contribute to the development of the care plan
• Stay up-to-date with the latest research and best practices in physical therapy, and apply this knowledge to improve patient outcomes

Requirements:

We welcome candidates with a Degree in Physical Therapy (PT) from an accredited institution and current licensure or certification as a Physical Therapist.
