About this role

Job description

Aviator Health supports older adults and patients with chronic conditions. Oxbow Health Inc. facilitates the connection between patients and Aviator Health by supporting the onboarding of qualified healthcare professionals.

Aviator Health is actively recruiting a Remote Telehealth Physician Assistant. As an Aviator PA, you will work with our physicians and health advocates to make sure patients receive coordinated, high-quality care. You’ll use your clinical expertise to help seniors and their families navigate chronic illness and support patients in achieving better health outcomes.

Position Overview

We are on a mission to give patients the care they deserve by making the healthcare system easy to navigate. American healthcare is so complex that 88% of Americans do not have the health literacy needed to navigate it.

Navigating healthcare today is overwhelming, especially for patients living with chronic conditions. At Aviator, we’re building a new model of support that combines human expertise with technology to make care more accessible and coordinated. Physician Assistants play a key role in delivering that care.

Responsibilities

  • Provide clinical care and expertise to support patients navigating chronic illness.
  • Partner with health advocates and supervising/collaborating physicians to build and carry out holistic care plans.
  • Identify and prioritize patient needs, ensuring continuity of care.
  • Integrate social determinants of health (e.g., food, transportation, housing) into patient plans.
  • Contribute to building Aviator’s patient care program, helping us refine and expand our model.
  • Use technology and AI-enabled systems to deliver care efficiently and effectively.
  • Maintain accurate, timely clinical documentation consistent with state practice requirements.

Qualifications

  • Graduate of an ARC-PA accredited Physician Assistant program.
  • Current NCCPA certification (PA-C).
  • Current, active PA license in multiple states; a minimum of 20 states is required (ideally licensed in 40+ states) and FL and TX are a must - we work with many patients in those states. Candidates licensed in more states are strongly preferred.
  • Open to obtaining additional state licenses, including through the PA Licensure Compact as it becomes available.
  • Experience in primary care, internal medicine, family medicine, or geriatrics, with a focus on patients with chronic conditions.
  • Telehealth experience preferred.
  • Strong clinical judgment and ability to coordinate across multiple providers.
  • Excellent communication and empathy skills; you listen first and guide second.
  • Comfortable with technology and able to quickly learn new systems.
  • A proactive, solutions-oriented mindset; you thrive in fast-paced, evolving environments.
