Aventura at Creekside in Carbondale, PA, is looking for an enthusiastic and compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team!

What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and independence
• Collaborate with nursing, therapy, and interdisciplinary teams
• Maintain accurate and timely documentation using Net Health and PCC
• Help residents achieve their rehabilitation goals and improve their quality of life

What We're Looking For

• Graduate of an accredited Physical Therapy program
• Current Pennsylvania Physical Therapist license
• Strong communication and teamwork skills
• Compassionate, motivated, and patient-focused approach
• SNF/LTC experience is a plus!
