Aventura at Carriage Inn is growing our in-house rehabilitation team!

We are seeking a motivated and compassionate Physical Therapist (PT) to join our team in Dayton, Ohio. Our primary need is for a Full-Time PT, with PRN opportunities also available.

Position Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide skilled PT services to our SNF/LTC population
• Help residents improve strength, mobility, balance, endurance, and overall functional independence
• Establish measurable goals and monitor resident progress
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Utilize Net Health and PointClickCare (PCC)
• Participate in discharge planning and resident/family education
• Maintain compliance with facility, state, and federal guidelines

Qualifications

• Graduate of an accredited Physical Therapy program
• Current or eligible Ohio Physical Therapist license
• Strong communication and interpersonal skills
• Passion for providing quality, resident-centered care
• SNF/LTC experience is a plus, but not required
• New graduates are encouraged and welcome to apply!
