About the Role

As a Physical Therapist at Astoria Skilled Nursing and Rehabilitation, you will have the opportunity to work with a diverse range of patients, from short-term rehabilitation to long-term care. You will design and implement effective treatment plans, assessing patients' abilities and developing strategies to improve their mobility, balance, and function.

Our state-of-the-art rehab-recovery suites are equipped with modern technology and provide a comfortable and private setting for our guests.

Responsibilities:

• Conduct thorough evaluations of patients' physical and functional abilities
• Develop and implement personalized treatment plans to achieve rehabilitation goals
• Collaborate with healthcare teams to ensure comprehensive care
• Provide patients with education and training on exercises, mobility, and self-care techniques
• Maintain accurate and detailed records of patient progress and treatment plans
• Participate in multidisciplinary team conferences to discuss patient care and progress
