Part-Time or PRN Physical Therapist – Home Health & Hospice

We are seeking a compassionate and dependable Physical Therapist to provide quality care to patients in their homes.

Responsibilities:

• Perform physical therapy evaluations and treatments.
• Develop individualized plans of care.
• Help patients improve mobility, strength, balance, and safety.
• Collaborate with patients, families, and the healthcare team.
• Complete documentation in accordance with agency and regulatory requirements.

Qualifications:

• Current, unrestricted Physical Therapist license.
• Strong communication and organizational skills.
• Ability to work independently and manage time effectively.
• Home health or hospice experience preferred.
• Valid driver's license and reliable transportation.

Pay: From $60.00 per hour
