Position Overview
The Registered Nurse (RN) Patient Care Coordinator serves as a clinical resource for patients receiving specialty pharmacy therapies and for the teams supporting their care. The RN helps patients transition into chronic therapy through a supportive start-of-care process and provides ongoing clinical support as therapy continues.
Patients may receive intravenous (IV) or injectable medications in an infusion suite or at home through contracted nursing agencies. The RN works with patients, caregivers, prescribers, pharmacy personnel, intake coordinators, refill coordinators, and nursing agencies to support understanding of the therapy process, identify clinical needs or concerns, and ensure appropriate communication and escalation.
Intake coordinators and refill coordinators are responsible for operational follow-up of referral and refill schedules, nursing-visit scheduling, and medication delivery coordination. The RN does not own those scheduling or delivery functions but may serve as a clinical resource when questions or concerns arise.
The RN performs duties within applicable licensure, scope of practice, organizational policies, and established clinical protocols.
Essential Duties and Responsibilities
Start-of-Care Support
- Provide a supportive introduction to the specialty therapy process for patients beginning chronic treatment.
- Contact patients, as appropriate, to explain the RN’s role, identify questions or support needs, and help them understand what to expect as they begin therapy.
- Reinforce clinical education provided by the prescriber, pharmacist, or nursing provider, within the RN’s scope and according to approved materials and protocols.
- Help identify patient-reported barriers, concerns, or unmet support needs that could affect their understanding of or participation in therapy.
- Communicate relevant findings and follow-up needs to the appropriate pharmacy, nursing, prescriber, or care coordination team.
Ongoing Patient Clinical
Support
- Serve as an accessible nursing resource for patients and caregivers during ongoing therapy, consistent with organizational workflows and established protocols.
- Respond to nursing-related questions within the RN’s role, and direct medication-specific questions, treatment-plan decisions, or other concerns to the pharmacist, prescriber, or appropriate clinician.
- Conduct follow-up calls when indicated by the care plan or organizational workflow to check on the patient’s experience with therapy and identify reported concerns or support needs.
- Recognize patient-reported symptoms or possible adverse reactions and promptly escalate them according to established clinical protocols.
- Reinforce instructions from the prescriber, pharmacist, or nursing provider without independently changing medication orders or treatment plans.
- Support clear, respectful communication with patients and caregivers, including appropriate handoffs when a concern requires another clinician or department.
Clinical Resource for Coordinators and Care Partners
- Serve as a clinical resource to intake coordinators, refill coordinators, and other team members when patient-related clinical questions or concerns arise.
- Help the coordinator team recognize when a patient issue requires nursing assessment, pharmacist or prescriber input, or escalation under established protocols.
- Review relevant clinical information or documentation when requested and within the RN’s assigned responsibilities.
- Communicate with contracted nursing agencies, prescribers, pharmacy staff, and other care partners regarding clinical concerns and follow-up needs, as appropriate.
- Collaborate with the care team to support continuity of care and clear communication throughout the patient’s therapy.
Escalation and Documentation
- Identify and promptly escalate patient-reported concerns, possible adverse reactions, changes in condition, or other issues that may affect safety or continuity of therapy.
- Route medication-related questions to the pharmacist and treatment-plan questions to the prescriber or appropriate clinician, following organizational protocols.
- Document patient contacts, education or support provided, reported concerns, clinical communications, escalations, and follow-up steps accurately and within required timelines.
- Share relevant clinical information with authorized team members to support timely follow-up and appropriate handoffs.
- Maintain confidentiality and follow applicable documentation, privacy, and recordkeeping requirements.
Quality and Workflow Development
- Collaborate with the Director of Nursing (DON), Compliance Department, pharmacy personnel, and coordinator teams to develop and refine workflows for patient start-of-care support, ongoing clinical outreach, and issue escalation.
- Identify recurring patient support needs or communication gaps and share opportunities for improvement with the appropriate leadership.
- Support adherence to physician orders, established clinical protocols, organizational policies, and applicable regulatory requirements.
Role Boundaries
- Intake coordinators manage operational referral follow-up.
- Refill coordinators monitor refill and therapy schedules and perform related follow-up.
- The appropriate coordinator or operations team manages nursing-visit scheduling and medication delivery coordination.
- The RN provides clinical support, education, assessment within scope, and escalation; the RN does not independently change medication orders or treatment plans.
Minimum Qualifications
- Current, active California Registered Nurse (RN) license in good standing.
- Experience in patient care, infusion therapy, specialty pharmacy, home infusion, or a related clinical setting.
- Experience communicating with patients and caregivers about ongoing treatment or care needs.
- Familiarity with IV or injectable therapies and the roles of specialty pharmacy, prescribers, and nursing providers.
- Ability to recognize concerns that require escalation and follow established clinical protocols.
- Strong communication, documentation, organization, and problem-solving skills.
- Ability to collaborate effectively with patients, coordinators, pharmacy staff, nursing agencies, prescribers, and clinical leadership.
Preferred Qualifications
