Job Summary

Apex Care LLC is seeking a qualified and compassionate Clinical Registered Nurse (RN) to support the delivery of safe, reliable, and person-centered home care services.

The Clinical RN will be responsible for conducting client assessments, developing and updating care plans, supervising home care staff, and monitoring the quality of services provided to clients. The successful candidate will work closely with agency leadership, caregivers, clients, and families to ensure that care meets individual needs and applicable professional and regulatory standards.

Key Responsibilities

• Conduct initial admission assessments and annual client reassessments.
• Develop, review, and update individualized client care plans.
• Perform supervisory visits and monitor the quality of care delivered in clients' homes.
• Assess and verify the skills and competencies of field employees.
• Supervise in-home aides and provide appropriate clinical guidance.
• Verify required tuberculosis screening documentation and staff credentials.
• Participate in quality assurance activities, including reviewing client records, incident reports, and complaints.
• Maintain accurate and confidential client records and documentation.
• Communicate with clients, families, caregivers, and agency leadership regarding care needs and concerns.
• Assist in addressing client complaints and resolving care-related concerns.
• Coordinate referrals with other healthcare professionals and service providers when necessary.
• Verify client service authorizations and ensure services remain within the approved scope of care.
• Participate in supervisory on-call responsibilities as assigned.
• Assist the Agency Director with clinical and administrative functions.
• Report relevant clinical and operational concerns to the Administrator or Agency Director.

Qualifications

• Associate degree in Nursing or higher from an accredited nursing program.
• Current, unrestricted RN license valid for practice in Virginia.
• Minimum of one year of professional experience as a Registered Nurse.
• Current CPR certification.
• Strong clinical assessment, care planning, documentation, and communication skills.
• Ability to supervise caregivers and maintain professional relationships with clients and families.
• Knowledge of patient confidentiality, infection prevention, and applicable healthcare standards.
• Ability to travel to client homes for assessments and supervisory visits as required.

Danville, Virginia, with visits to client homes as required.
