Physical Therapist- Home Health Care

Overview

Amore Home Health Care is seeking a Physical Therapist to join our team. We are looking for someone to specifically do new patient admission assessments and plan of care development (not Oasis, nursing does this), 30 day re-evals, and discharges, while our PTAs will provide the weekly treatments.

We have the flexibility to group visits to 1 day a week, 1 day every other week, or even 1 day a month depending on clinician’s preference. We are a nurse-owned and nurse-operated company, as well as family-focused, meaning we understand the importance of having a schedule that works for you.

Our home office is located in Middleboro, MA, and we service patients in Plymouth, Bristol, Barnstable, and Norfolk counties. We use Alora EMR software which is known for its user friendliness, reducing time spent on documenting.

This is a W-2 position. On boarding is 1 day in office, rest out in field. We recognize you do not need someone to orient you to being a therapist, just someone to assist with the electronic medical records system.

There will be a supervisor with you for admissions to assist, as well as available for assistance during any other visits as you get familiar with the EMR.

Pay is per visit as follows

Admission with plan of care development: $175

30 day re-eval: $100

Discharge: $125

Milage is paid between clients at $0. 73/mile.

Duties

• Conduct admission assessment (no OASIS- nursing does these) to evaluate patients' physical conditions and develop personalized treatment plans.
• Over see PTAs care being provided and re-evaluate patient's at 30 day mark.
• Complete a discharge assessment and summary when patient's have met their goals.
• Work with diverse patient populations including orthopedic, geriatric, and acute care cases.
