Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments in patients' homes.
• Develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled physical therapy to improve mobility, strength, balance, and functional independence.
• Perform reassessments and discharge evaluations as appropriate.
• Educate patients and caregivers regarding home exercise programs, fall prevention, and safe mobility.
• Maintain accurate and timely clinical documentation.
• Collaborate with nurses, physicians, and other healthcare professionals to coordinate patient care.

Qualifications

Required:

• Current, active California Physical Therapist license
• Current CPR/BLS certification
• Valid driver's license and reliable transportation
• Strong communication and organizational skills
• Ability to work independently in a home health environment

Preferred:

• At least one year of physical therapy experience
• Previous home health experience
• Familiarity with electronic medical record systems and home health documentation
