Pay: $90. 00–$140. 00 per hour

Schedule: Full-time, part-time, and per diem

Work Location: Patient homes throughout the service area

Responsibilities

• Assess patients’ physical functioning, mobility, and therapy needs
• Develop and implement individualized treatment plans to restore, improve, or maintain physical functioning
• Provide compassionate, patient-centered physical therapy in patients’ homes
• Monitor progress and adjust treatment plans as appropriate
• Educate patients and caregivers on exercises, safe mobility, and home care strategies
• Complete clinical notes and treatment records accurately and on time
• Collaborate with physicians, nurses, and other care team members to support patient care goals

Qualifications

• Active California Physical Therapist license required
• Current CPR/BLS certification
• Valid California driver’s license and reliable transportation
• At least one year of experience in a home health or clinical setting preferred; new graduates welcome
• Strong communication, organization, and time management skills
• Ability to work independently and coordinate effectively with the clinical team
