Duties

• Conduct comprehensive patient assessments to determine functional limitations and physiotherapy needs in home and community settings
• Develop and implement individualized care plans that incorporate evidence-based practices such as trigger point therapy, orthopedics, and geriatrics care
• Perform therapeutic interventions including manual therapy, exercise programs, mobility training, and pain management techniques aligned with patient goals
• Coordinate discharge planning with healthcare teams to ensure seamless transition of care from inpatient or outpatient facilities to home environments
• Document patient progress accurately using electronic medical record (EMR) systems, ensuring compliance with medical terminology standards and privacy regulations
• Collaborate with multidisciplinary teams to optimize care strategies for patients across acute care, hospital experience, outpatient clinics, and home health environments
• Educate patients and caregivers on physiotherapy exercises, safety precautions, and health maintenance strategies to promote long-term wellness

Qualifications

• Valid licensure as a Physical Therapist with relevant state or regional licensing boards
• Demonstrated experience in home & community care settings with a strong understanding of physiology and anatomy knowledge
• Proven ability to develop comprehensive care plans for diverse populations including pediatrics, geriatrics, orthopedics, and outpatient clients
• Familiarity with EMR systems for documentation and patient management
• Knowledge of medical terminology relevant to physiotherapy practices in inpatient, hospital, outpatient, and clinic environments
• Experience in discharge planning and working within multidisciplinary teams to ensure continuity of care
• Strong communication skills to effectively educate patients and collaborate with healthcare professionals

Pay: $75.00-$90.00, per visit.
