We are a growing, locally operated home health care organization seeking compassionate, dependable, and experienced Registered Nurses (RNs) to provide skilled nursing services in Toledo, Ohio, and surrounding communities.

Compensation

• Case Openings / Start-of-Care Admissions: $50–$60 per visit
• Recertification Visits: $45–$55 per visit
• Other skilled nursing visits: Compensation based on visit type.
• Mileage reimbursement available in accordance with company policy.

Key Responsibilities

• Complete skilled nursing visits, patient admissions, and recertification assessments as assigned and qualified.
• Perform comprehensive nursing assessments in patients' homes.
• Complete OASIS assessments when required.
• Review medications, assess patient safety, and identify changes in patient condition.
• Develop and update individualized nursing care plans.
• Provide skilled nursing treatments, wound care, and medication management within the RN's scope and demonstrated competencies.
• Educate patients and caregivers regarding medications, disease management, safety, and treatment plans.
• Communicate with physicians and interdisciplinary team members regarding patient needs.
• Complete accurate and timely clinical documentation.
• Coordinate effectively with scheduling personnel and clinical management.
• Follow applicable Medicare, Medicaid, Ohio nursing, and home health regulations.
• Provide respectful, culturally responsive care to patients and families from diverse backgrounds.

Qualifications

Required

• Active, unrestricted Ohio Registered Nurse (RN) license.
• Strong nursing assessment, documentation, and communication skills.
• Reliable transportation and willingness to travel within assigned service areas.
• Ability to manage patient visits independently within demonstrated clinical competencies.
• Dependable availability and commitment to completing assigned visits and documentation on time.
• Professional and compassionate approach to patient care.

Preferred

• Previous Medicare-certified home health nursing experience.
• Experience completing OASIS assessments, start-of-care admissions, and recertifications.
• Familiarity with Medicare home health documentation requirements.
• Experience with Alora or similar electronic medical record systems.
• Experience with wound care, chronic disease management, and geriatric nursing.
