Physical Therapist - PRN-Home Health

Job Overview

As a PRN Home Health Physical Therapist, you’ll provide individualized physiotherapy to patients in their homes and communities. You’ll assess mobility and function, develop treatment plans and care plans, and support patient rehabilitation toward practical daily goals. Apply your knowledge of anatomy and physiology to deliver thoughtful patient care across a range of needs.

Responsibilities

• Evaluate patients and develop, implement, and update treatment plans based on their condition, goals, and progress.
• Provide physical therapy interventions, including therapeutic exercise and trigger point therapy when appropriate.
• Coordinate care with healthcare professionals, patients, and caregivers; contribute to discharge planning.
• Document assessments, treatment, and progress in electronic health records (EHR) or electronic medical record (EMR) systems, following clinical documentation standards.

Requirements

• Current authorization to practice in a physical therapy role, as required for the position and applicable jurisdiction.
• Knowledge of medical terminology, anatomy, physiology, physical therapy documentation, and patient rehabilitation.
• Experience with home health or home and community care is relevant; acute care, inpatient, hospital, clinic, outpatient, or outpatient rehabilitation center experience is also valuable.
• Familiarity with EHR/EMR management and clinical documentation standards.
• Experience with geriatric patients or pediatrics, and exposure to orthopedics or neurology, is welcome. Applicants may also bring outpatient experience and client communication skills.

Pay: $125.00 per hour

License/Certification:

• Physical Therapy License (Required)
