Job Overview

We are seeking a Licensed Physical Therapist to provide thoughtful, evidence-informed care that helps patients improve mobility and function. This role includes assessing patient needs, developing individualized care plans, and supporting rehabilitation across relevant care settings. The successful candidate communicates clearly with patients and collaborates with healthcare professionals to coordinate care.

Duties

• Evaluate patients’ movement and functional needs using knowledge of anatomy and physiology, and explain findings using appropriate medical terminology.
• Develop and adjust treatment plans, deliver physical therapy interventions—including trigger point therapy when appropriate—and document care in accordance with clinical documentation standards.
• Your impact: Coordinated treatment and discharge planning help patients and care teams make informed decisions that support safe transitions and continued rehabilitation.
• Communicate with patients and caregivers, monitor progress, and coordinate patient care with healthcare professionals.
• Maintain accurate physical therapy documentation and manage records in electronic health records (EHR) or electronic medical record (EMR) systems.

Experience

• Current physical therapy licensure and experience practicing in a physical therapy role are required.
• Acute care, inpatient, hospital, clinic, outpatient clinic, outpatient rehabilitation center, home health, or home and community care experience is relevant.
• Experience with patient rehabilitation, orthopedics, neurology, geriatrics, geriatric patients, or pediatrics is valued.
• Familiarity with care plans, discharge planning, client communication, and EHR/EMR management is preferred.
• Demonstrated knowledge of physiotherapy, patient care, and physical therapy documentation is essential.

Pay: $45.00 - $55.00 per hour
