Overview

We are seeking a Per Diem or Full Time dedicated and skilled Physical Therapist specializing in Home Health Care to join our dynamic team. The ideal candidate will have extensive experience in outpatient and inpatient settings, with a focus on patient rehabilitation and community-based care.

This role offers an opportunity to make a meaningful impact by developing personalized treatment plans and collaborating closely with healthcare professionals to promote optimal patient outcomes.

Responsibilities

• Conduct thorough assessments of patients' physical conditions, including mobility, strength, and functional limitations.
• Develop individualized care plans tailored to each patient's needs, including goals for recovery and independence.
• Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology, pediatrics, and geriatrics.
• Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care.
• Provide education and training to patients and their families on exercises, safety measures, and community resources for ongoing support.

Skills

• Proven experience practicing in a physical therapy role within Home Health or outpatient clinics.
• Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy.
• Experience working with geriatric patients and those requiring outpatient rehabilitation services.
• Proficiency in managing electronic medical records (EMR) systems for documentation purposes.
• Ability to develop effective treatment plans based on comprehensive assessments and clinical evidence.
• Excellent client communication skills with a compassionate approach toward patient care.
