About the Position

Agnes & Sandy’s Caring Hands is seeking an experienced Registered Nurse to support client assessments, care planning, caregiver competency evaluations, clinical oversight, and regulatory compliance.

We specialize in compassionate home care for individuals living with dementia, Alzheimer’s disease, and memory loss. The ideal RN brings extensive home care experience and a strong working knowledge of Georgia’s Private Home Care Provider regulations and Department of Community Health requirements.

Critical Availability Requirement

ASCH requires availability to complete new client assessments within 24 hours of receiving a referral, including flexibility for urgent admissions. Applicants must clearly describe their availability, service areas, and ability to meet this requirement.

Responsibilities

• Complete initial and ongoing client assessments.
• Develop, review, and update individualized care plans and service plans.
• Conduct RN supervisory visits and complete timely, accurate documentation.
• Support clinical onboarding and evaluate caregiver competency through observation and skills demonstrations.
• Identify client-specific training needs and provide appropriate caregiver instruction.
• Review clinical and client records for completeness, accuracy, and compliance.
• Help develop and maintain home care policies and procedures.
• Support DCH survey readiness, internal reviews, and corrective actions.
• Communicate changes in client needs to the administrator and appropriate care team members.
• Work independently while maintaining professional communication and accountability.

Required Qualifications

• Current, unrestricted RN license or authorization to practice in Georgia, subject to verification.
• Extensive home care nursing experience.
• Strong knowledge of Georgia Private Home Care Provider regulations, DCH requirements, and documentation standards.
• Experience with assessments, care planning, supervisory visits, and caregiver competency evaluations.
• Ability to meet ASCH’s 24-hour new client assessment requirement.
• Excellent clinical judgment, organization, written documentation, and communication skills.
• Reliable transportation to client locations.
• Current CPR and First Aid certification.
• Completion of applicable screening and onboarding requirements.

Preferred Experience

• Home care administration or nursing leadership.
• Clinical oversight of a private home care agency.
• Regulatory compliance, survey preparation, or corrective action planning.
• Dementia and Alzheimer’s care.
