Responsibilities

• Perform comprehensive physical therapy evaluations in the home setting
• Develop individualized treatment plans based on patient goals and clinical needs
• Provide skilled physical therapy services in patients' homes
• Help patients improve strength, mobility, balance, endurance, and functional independence
• Assess home environments and incorporate them into treatment plans to maximize outcomes
• Educate patients and caregivers on exercises, safety techniques, equipment use, fall prevention, and injury prevention
• Collaborate with physicians, nurses, occupational therapists, speechlanguage pathologists, and other healthcare professionals
• Monitor patient progress and modify treatment plans as needed
• Maintain accurate and timely clinical documentation
• Complete patient notes and required communications within agency timeframes
• Ensure compliance with agency policies and applicable regulations
• Travel to patient homes within assigned coverage areas

Qualifications

• Active Pennsylvania Physical Therapist (PT) License
• Graduate of an accredited Physical Therapy program
• Home Health experience preferred but not required
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal skills
• Strong computer and documentation skills
• Reliable transportation and valid driver's license
• Ability to work independently and effectively manage a flexible schedule
• Knowledge of electronic medical record systems is a plus
• Commitment to delivering exceptional patientcentered care
