About this role

Addy’s Colors Collaborative Care Clinic | Gloucester, Virginia

Location: Newport News, Virginia Position: Part-Time or Full-Time Salary: $60-75 an hour; $110,000–$140,000 annually, based on experience and qualifications Schedule: Monday–Friday Benefits: Competitive benefits package

Who We Are

Addy’s Colors Collaborative Care Clinic is a growing 501(c)(3) healthcare organization built around a simple belief:

People deserve to be cared for as whole people—not as a diagnosis, a symptom, or a medical chart.

Our collaborative care model brings primary care and behavioral health together to create a more connected healthcare experience for children, adults, and families.

We are a mission-driven organization that values Authenticity, Co-Regulation, Connection, Mindfulness, and Ubuntu. These values are not simply words on a wall—they influence how we care for our patients, communicate with one another, and approach our work.

We are looking for a Family Nurse Practitioner who wants to be part of building something meaningful in the Gloucester community.

The Opportunity

We are seeking a Family Nurse Practitioner who is passionate about primary care and believes that good healthcare begins with relationships.

This is an opportunity for an FNP who wants more than a traditional provider role.

You will have the opportunity to develop meaningful relationships with your patients, practice comprehensive primary care, collaborate closely with behavioral health professionals, and contribute to the continued development of our collaborative care model.

Our ideal provider is clinically strong, compassionate, dependable, and comfortable working in a growing organization where collaboration and flexibility are important.

What You Will Do

As a Family Nurse Practitioner at Addy’s Colors, you will:

  • Provide comprehensive primary care to patients across the lifespan.
  • Perform medical histories, physical examinations, assessments, and clinical evaluations.
  • Diagnose and treat acute and chronic medical conditions within your scope of practice.
  • Develop individualized treatment plans in partnership with patients and families.
  • Provide preventive care and health maintenance.
  • Manage chronic health conditions and monitor treatment outcomes.
  • Prescribe medications and treatments in accordance with Virginia law and your scope of practice.
  • Order and interpret appropriate laboratory and diagnostic testing.
  • Provide health education and counseling.
  • Make appropriate referrals and coordinate care with outside specialists.
  • Follow patients throughout their care journey rather than treating each visit as an isolated encounter.
  • Maintain timely, accurate, and clinically appropriate documentation.
  • Participate in quality improvement and organizational initiatives.
  • Communicate effectively with patients, families, staff, and other healthcare providers.
  • Work collaboratively with Addy’s Colors' behavioral health and care coordination teams.

Collaborative Care Is at the Heart of What We Do

At Addy’s Colors, primary care does not operate in a silo.

Our providers work alongside therapists, behavioral health professionals, care coordinators, and other members of the healthcare team to identify barriers to wellness and develop coordinated approaches to care.

You will have the opportunity to collaborate with professionals who may be working with the same patient from different perspectives.

We believe that sometimes the most important part of healthcare is not simply asking, "What is wrong?"

It is asking:

"What does this person need?"

What We Are Looking For

We are looking for someone who combines clinical competence with genuine compassion.

Our ideal FNP:

  • Enjoys developing long-term relationships with patients.
  • Practices patient-centered, evidence-informed care.
  • Is comfortable caring for patients across the lifespan.
  • Communicates clearly and respectfully.
  • Values collaboration rather than working independently from the rest of the team.
  • Is dependable and accountable.
  • Can balance clinical responsibilities with the administrative responsibilities of outpatient practice.
  • Is comfortable working in a growing organization.
  • Is willing to contribute ideas and help improve systems and processes.
  • Understands the importance of timely documentation and follow-through.
  • Treats colleagues with the same respect and compassion expected in patient care.
  • Is committed to creating an environment where patients feel heard, respected, and safe.

Qualifications

Required

  • Master's degree or higher from an accredited Nurse Practitioner program.
  • Current, unrestricted Virginia Nurse Practitioner license.
  • National certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
  • Current DEA registration or eligibility to obtain one.
  • Current BLS certification.
  • Experience with mental health medications
  • Ability to practice independently within applicable Virginia laws and regulations.
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication and interpersonal skills.

Preferred

  • VMAP Certified and/or PMHNP
  • Experience in family practice or outpatient primary care.
  • Experience caring for patients across the lifespan.
  • Experience with chronic disease management.
  • Experience working with underserved or rural communities.
  • Experience in an integrated or collaborative care environment.
  • Interest in whole-person healthcare.

What Makes This Position Different?

You will not simply be filling an exam-room schedule.

You will be joining a team that is actively creating a different kind of healthcare experience.

At Addy’s Colors, we want our patients to know their provider, feel comfortable asking questions, and understand that their physical and emotional wellbeing are connected.

We also want our providers to feel that their work has meaning.

You will have the opportunity to

Build relationships. Know your patients beyond their diagnoses.

Collaborate. Work alongside behavioral health and care coordination professionals.

Grow. Develop professionally as our organization continues to expand.

Make an impact. Help improve access to quality healthcare in the communities we serve.

Be part of something bigger. Contribute to a nonprofit organization whose mission extends beyond simply providing medical services.

Our Values

Authenticity: We show up as ourselves and create space for others to do the same.

Co-Regulation

We understand that how we show up for one another matters. Calm, connection, and safety influence healing.

Connection

Relationships are central to effective healthcare.

Mindfulness

We strive to be present, intentional, and thoughtful in how we care for patients and one another.

Ubuntu: "I am because we are." We believe that healthcare is stronger when we work together.

Compensation & Benefits

Salary: $110,000–$140,000 annually, based on experience, qualifications, and overall fit for the position.

Addy’s Colors offers a competitive benefits package and a supportive, collaborative work environment.

Additional opportunities may be available as the organization grows.
