About this role
Overview
The Physical Therapist (PT) provides evaluation, treatment planning, and hands-on rehabilitative care to an adult orthopedic and sports population at Achieve Strength, Achieve Health & Wellness's orthopedic branch. This role serves patients across the full spectrum of activity levels — from weekend warriors and active adults recovering from injury or surgery, to competitive and professional athletes returning to sport.
The PT is expected to deliver evidence-based, individualized care consistent with The Achieve Way (Compassion, Learning, Evidence-Based Practice, Communication, Community) and to contribute to a positive, high-performing clinic culture.
Duties
- Conduct comprehensive initial evaluations for orthopedic and sports-related conditions, including post-surgical, post-injury, and performance-based referrals.
- Develop individualized, goal-oriented plans of care using evidence-based methods and appropriate outcome measures.
- Provide direct, hands-on treatment including therapeutic exercise, manual therapy, neuromuscular re-education, and functional/sport-specific training.
- Progress and modify treatment plans based on patient response, re-evaluation findings, and functional goals.
- Maintain timely, accurate, and payer-compliant documentation in Prompt EMR for evaluations, daily notes, progress notes, and discharge summaries.
- Communicate proactively with referring physicians, surgeons, athletic trainers, and other members of the care team regarding patient progress and return-to-activity/return-to-sport timelines.
- Educate patients and, when appropriate, coaches or family members on injury prevention, home exercise programs, and self-management strategies.
- Supervise PTAs, aides, or students as assigned, in accordance with Minnesota PT practice standards.
- Participate in clinic meetings, in-services, and continuing education to stay current on best practices in orthopedic and sports rehabilitation.
- Support scheduling and caseload management practices that reflect efficient use of clinic capacity.
- Uphold all HIPAA, payer, and Achieve Health & Wellness compliance and documentation standards.
Experience
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
- Current, unrestricted Minnesota PT license (or eligibility to obtain prior to start date).
- Current CPR/BLS certification.
- Approximately 2–5 years of clinical experience in orthopedic and/or sports rehabilitation preferred.
- Strong manual therapy and clinical reasoning skills; comfort treating a wide range of orthopedic diagnoses and activity levels.
- Specialty certifications a plus but not required, e.g., OCS, SCS, dry needling, blood flow restriction (BFR), or sports residency training.
- Excellent communication skills and a collaborative, team-oriented approach.
- Familiarity with EMR documentation systems; Prompt EMR experience a plus.
